Topic: Distraction Osteogenesis Maxillary Expansion (DOME) for adult obstructive sleep apnea patients combined with ENT Surgery (40 MIN)
Abstract
Maxillofacial and oropharyngeal anatomy are critical contributors to the pathophysiology of obstructive sleep apnea (OSA). Maxillary constriction with a high arch palate is associated with high nasal airway resistance. Coupled with decrease in oral cavity volume, transverse deficiency of the maxilla is known as a contributor to the development of OSA. DOME is a two-step process starting with insertion of custom-fabricated maxillary expanders anchored to the hard palate by mini-implants followed by minimally invasive osteotomies. After maxillary expansion will complete, orthodontic treatment to restore normal occlusion will be initiated. In many patients there is also the need for Septoplasty and/or Turbinoplasty- as suggested by an ENT surgeon. This situation calls for an additional surgery that the patients may have to undergo in future. Therefore combining these two procedures i.e the ENT and DOME procedure is the most effective way for correction of the Oro-nasal compromise.
